Stick the original screws through the M42 adapter and the two plastic spacers. Then screw the whole thing onto the lens. Now you're basically finished!
To reach inifinity focus it might be necessary to grind away a little from the plastic spacer ring thickness or alternatively from the M42 adapter surface.
